To help with referrals we have created some pdf referral proformas which you are welcome to print out and send to us by post. Radiographs can be sent in digital format to info@berkhamsteddental.com if you prefer. Please note this is not an encrypted email account. For data security please do not send patient sensitive information by email. Radiographs should be identified by pseudonyms such as initials with a cross reference on the written referral.
