Save your natural tooth. It’s worth fighting for

root canal treatment

Root canal treatment has a reputation that bears very little relation to reality. Ask most people who have had one, and they will tell you the same thing: it was far more straightforward than they expected. Modern root canal treatment, carried out under effective local anaesthetic, is no more involved or uncomfortable than having a filling, and it is one of the most important treatments available for saving a tooth that might otherwise be lost.

The pulp is the soft tissue at the centre of the tooth, containing nerves and blood vessels. When it becomes infected or damaged, usually as a result of deep decay, a cracked tooth or repeated dental work, it can cause significant pain and, if left untreated, can lead to abscesses or tooth loss. Root canal treatment removes the infected or damaged pulp, cleans and shapes the canals inside the root and seals the tooth to protect it from further infection. The tooth itself is preserved, restored and in most cases, functions normally for many years afterwards.

At Berkhamsted Dental Practice, straightforward root canal treatment is performed by our experienced dental team. For complex cases, including difficult canal anatomy, retreatment and situations where specialist precision is essential, patients are referred to Dr Philip Mitchell, our in-house specialist in endodontics. Having a specialist of Dr Philip’s calibre available on-site means even the most demanding cases are managed with the highest level of clinical expertise, without the need to refer elsewhere.

Treatment Information

When is it needed?

Some signs that root canal treatment may be needed are obvious. Others are not. Many infections are caught on X-ray at a routine check-up, long before any discomfort develops. If you notice any of the following, contact us without delay.

Note: Some people experience no symptoms at all. Infection can be detected through an X-ray at a routine examination, which is one of many reasons regular check-ups matter.

Especially when biting, chewing or applying pressure to the tooth. Pain that does not settle is rarely something to wait on.

When temperature sensitivity continues long after the source has been removed, the nerve inside the tooth may be compromised.

Discolouration can be a sign of nerve damage or internal tissue change within the tooth.

Localised swelling near a tooth or tenderness along the gum line can indicate an infection spreading from the root.

A recurring spot or blister near a tooth is a sign that infection is present and needs treatment promptly.

Teeth that have undergone multiple restorations or that have suffered extensive decay are more vulnerable to pulp infection.

root canal procedure

Your Implant Journey

From consultation to a confident new smile.

journey-process-consultation

Consultation

Your dentist will examine the tooth carefully, take an X-ray or 3D CBCT scan to assess the extent of the infection. They will explain the procedure fully, answer your questions, and ensure you feel completely informed and comfortable before anything begins.

journey-process-preparation

Preparation

The area is numbed with a local anaesthetic to allow your dentist to access the pulp chamber, remove the infected or damaged tissue, and carefully clean and shape each canal using fine precision instruments.

Treatment

The canals are then disinfected and filled with a biocompatible material called gutta-percha, which seals the tooth against re-infection. At the end of treatment, a temporary filling is placed to protect the tooth while it heals.

Results

Once the tooth has healed, a long-lasting restoration, most commonly a crown, is fitted to protect and restore the tooth to full function. This is an important step: a crown provides the structural support it needs to last for many years.

Transparent pricing, exceptional value

Fees & pricing.

Root canal treatment fees vary depending on the complexity of the case, the number of canals within the tooth, and whether the treatment is a first-time procedure or a retreatment.

Where a specialist referral to Dr Philip Mitchell is required, this will be reflected in the fee. In all cases, your treatment plan and full fee breakdown will be discussed with you in advance; there will never be any surprises. We offer complete transparency at every stage.

Frequently asked questions

How do I know if I need a root canal?

Common signs include a persistent toothache, sensitivity to heat or cold that lingers, a discoloured tooth, swelling near the gum, or a visible abscess. However, some infections cause no noticeable symptoms at all and are only detected on X-ray.

If you are experiencing dental pain of any kind, we would encourage you not to wait; book an appointment so we can assess the situation and advise you appropriately.

Why is saving the natural tooth better than extracting it?

Your natural teeth are always the best option when they can be preserved. Nothing artificial replicates the function, feel, and structural benefit of a healthy natural tooth.

Losing a tooth can affect the surrounding teeth, the bite, the jawbone, and long-term oral health. Root canal treatment, followed by a crown, can restore a tooth to full function for many years, sometimes for life.

What happens during an endodontics consultation?

Your consultation begins with a conversation about your symptoms, how long you have had them, and any relevant dental history. Your dentist will then carry out a clinical examination of the affected tooth, which typically includes a series of simple tests to assess the health of the nerve, such as applying a cold stimulus or gentle pressure, and taking X-rays to examine the root canals and surrounding bone.

In some cases, a CBCT scan may be recommended to provide a more detailed three-dimensional view of the root structure. By the end of the appointment, you will have a clear diagnosis, an honest explanation of what is happening inside the tooth, and a full understanding of the treatment options available to you, including what happens if the tooth is left untreated. There is no obligation to proceed with anything on the day.

Is root canal treatment painful?

This is the question we hear most often, and the honest answer is that modern root canal treatment is very rarely painful. The procedure is carried out under local anaesthetic, which numbs the tooth and surrounding area completely.

Most patients are genuinely surprised by how comfortable the experience is. Any mild soreness afterwards is typically short-lived and easily managed with over-the-counter pain relief.

How long does root canal treatment take?

A straightforward root canal on a single-canal tooth (such as a front tooth) can often be completed in a single appointment of around 60 to 90 minutes. More complex teeth with multiple canals, such as molars, or cases requiring specialist input may take longer and may require two appointments. Your dentist will give you a realistic time estimate at your assessment.

What happens if I don't have root canal treatment?

Without treatment, an infected tooth will not heal on its own. The infection can spread, cause increasing pain, and ultimately result in an abscess or the loss of the tooth altogether.

Root canal treatment is almost always preferable to extraction; it saves your natural tooth, maintains the surrounding bone structure, and avoids the need for an implant or bridge to fill the space.

Will I need a crown after root canal treatment?

In most cases, yes. A tooth that has undergone root canal treatment has had its internal structure altered, and it can become more brittle over time. A crown provides the protection and structural support needed to prevent fracture and ensure the tooth lasts as long as possible. We will discuss the most appropriate restoration for your tooth at your appointment.

When would I be referred to Dr Philip Mitchell?

Dr Philip Mitchell, our specialist endodontist, manages cases that require an elevated level of complexity or precision. This includes teeth with unusual or difficult canal anatomy, retreatment cases where previous root canal treatment has not resolved the issue, and cases where specialist assessment is needed before a decision on treatment is made.

In all cases, you remain under the care of the practice and will be seen in a familiar environment.

Very happy with my Invisalign process with Kish. He kept me well informed throughout and always took time to answer all my many queries. The price was reasonable compared to others in the area and it has all been completed within the original time advised. I'm yet to have my composite bonding done but hope to have a transformed smile in the end. Would thoroughly recommend BDP if you're considering Invisalign.

I have just completed the main part of my Invisalign journey followed by composite bonding and could not be happier. Having been told by my previous dentist that it wasn’t worth doing ( due to my age and small change I would achieve) I had a consultation with Dr Kish to start a programme of Invisalign to straighten out my front teeth which had moved over time and now had a very slight overlap. Once complete, I saw Dr Ventsi for bonding to the edges of 6 teeth, which, as a heavy tooth grinder, were chipped and uneven. Both Drs were knowledgeable and lovely to deal with. I could not be happier with the result! A little change has made a huge difference !
